Because if you have a strong niche you can make big $, but being a very niche oriented yahoo publisher I can say that the pay is not that much better then adsense - at least as of lately. Targeting is also horrible, and the fear of being terminated for traffic quality is also not so plesent.

I see that it says I can "Can I place the ads on as many domains as I want to", does this mean once I have an account, I can use it on other sites/domains? Or do I have to get approval prior? I'm confused (as usual)

You Can, Provided that you should not show both the ads on a same page. It is against their TOS.

Nope, you can't have other network ads on your site together at the same time as YPN ads. But you can alternate one at a time though.

I've been wondering for a while now when they will open this up for non-US webmasters. They're no longer saying sometime in 2006, as you might imagine. But I don't know if I'll ever get in anyway.

From the Yahoo FAQ, they don't allow:

Interpreted broadly, that could be almost anything. The human suffering bit, anyway. Blogging that you broke your arm, sites about dentistry, almost any medical topic, mentioning the passing away of someone famous, and all kinds of news. What about debt, isn't that human suffering? They should be a lot more specific about where they draw the line on that one.

Companies like to leave their TOS as broad as possible so they can ditch you for whatever reason they like
