Try the 3rd (NeverBlueAds) and 4th (AffiliateBot) link in my signature. Both have plenty of leads based programs. AffiliateBot will accept you pretty fast and so will NBA if you fill in the correct information (i.e. Phone Number) so they can call you. Nothing difficult, they just want to make sure that you are a real person. I am very happy with both of these programs...
