[WTH] Someone to fix my CSS/PHP/JS error in Wordpress. (Looking for quick help)

    Hi,

    I am working on a new WordPress design and am currently trying to implement a contact form script I purchased (View FORM Here).

    The problem is that I can't seem to get the css to carry across and the inputted Google maps data to come across from the script to the page.

    Here is my effort so far

    I am looking for someone who can fix my mistakes and get it not only working properly, but also looking like the example.

    If you think you can do this, then contact me with a price to fix (all offers without a quote will be ignored) and I will give you more details/the files.
