Wth: Small job adding a contact form to my site.

    Hey,
    A really small job. I have the the contact form ready and its working but I need someone to just add it to my site so it looks good.

    Also adding mabye 3 more text fields to the form..

    Really easy job for the right guy. Should go fast so im not looking to pay more then $5 for this.
