I'm looking for someone to create a ajax "lightbox" or javascript box that will popup after the video plays using a player such as JWPlayer or whatever. Basically, after the video is over, this will popup and allow me to enter a php page with a form to collect name and email, similar to this: http://screencast.com/t/STMufsffS Ideally need this to be unique for each video, but do not need any type of UI. Just the script built so after the video is done playing or at a certain time it then pops this up. Please email me with questions, looking to have this done soon as the last developer left us hanging and didn't finish the job
