[WTH] Need someone to write a script

I need to get a script written that will work like this

Use will be shown a submission page with 3 fields and a drop box
information will be stored in mysql (preferred) or text file
Admin panel with login and logout ability and a page that will show all pending submissions and approved submissions. Need to be able to edit/approve/delete submissions. Also a way to add more categories for the drop down box on the submission page.
