WTB: Web Page Design

    Hello,

    I need a web page designed to go on my web site. I am looking for a Request a Quote page for my site http://tinyurl.com/mfqrot that looks similar to this site http://tinyurl.com/ksj7cp.

    I need to know how much that would cost. Please PM me a quote and how fast you could get it done. Seems like it should be relatively simple to me.

    Ok here is what I need. A form created in HTML I can copy and paste into my already created HTML/CSS page. The form should look like this and email the information to an email address when submit is clicked.
