[WTB] Simple Paypal IPN Form PHP

    Need a Paypal Form which uses IPN Details:

    Requirements:

    Form will have 2 visible fields :
    1.Drop down menu which lists 1 Item (for now).
    2. Input Text field called HIW Serial Number
    - Form Validation for the Serial Number Input field needed (Length = 31 characters) if the serial is not entered the user can not progress to paypal.

    Example Serial Format: 4B414B7A313E72614D72 - 1B390D37

    IPN:
    Upon successful payment, with a serial entered we must automatically add the buyers paypal email and serial # to the database. The Form must execute add.php?name=PAYPAL_EMAIL&uid=SERIAL only if the paypal transaction was successful
