WTB: Programmer to Copy a Form2Mail form (php)

    I need the form that is right here copied and made in php:

    http://grantreviewteamonline.com/business/

    Basically it's a form that needs to be able to send all the info and one file via e-mail. But, before the info is sent the payment via paypal has to be made.

    Let me know about how much it would cost for you to make this.

    I know there are free scripts for most of this but the paypal thing I need help with (plus I don't have the time).
