WTB: Need fancy contact form made

Hi,

I need a form made for my company. The forms need to use PHP to process the infomation.

I'd prefer someone that has created a few forms in the past as this one needs a few advanced features, such as another form coming up when something is selected.

The good thing about this project is that I already have a site with a form that mine needs to be the same as, which will obviously make things a lot easier.

Payment will be made by PayPal or NoChex on complection of form.

It seems okay but its a subscription service and looks like they host the forms. Not really what i'm looking for but thanks anyway.

 

Had two replies so far but they seem to be unsure.. I have the URL here with exactly what I need and have money ready in my PayPal account to send over.

This job should take now more than 2 hours max.
