WTB - need employment application form in html

    I have a paper based employment application which is a generic form that I need to have converted into an html form that looks, as closely as possible, the original. I don't need any javascript nor do I need it to post anywhere. I will take care of the back end data.

    If you're interested, send a pm and I"ll send the form in pdf so you can give me your quote. Payment will be upon completion of the work.

    I've had a significant number of responses and I've got a DP member that has committed to doing this for me. I've got everyone else's responses and if the current coder does not deliver, I'll pick someone else.
