You must edit your description and keywords on each post's edit page.

You are not making it clear as to what problem you are facing. But in case you are looking for page title to change on every page as per the thread title then you can go for a wordpress SEO plugin like Wordpress SEO by Yoast or Headspace 2. But I doubt that the problem is with your theme or you have modified the "wp title" section of your theme for which it is showing the same otherwise it should at least show the respective post names as the page title instead of the website name.

Use the All in One SEO Plugin. You can set meta tags on your posts and pages as you write your excerpts

For an example, check out the meta tags in the source code of my newest Wordpresss site:

As above, the all in one seo plugin is easiest to use (in my opinion)

When you create new posts you can change titles, descriptions and keywords, etc - it sounds like the current plugin only uses the global settings.

True. All you really need for Wordpress plugins is 1) All in One SEO Pack and 2) Google XML Sitemaps. Everything else just dependents on the style of site you want to make.
