    Does anyone know of a wordpress video testimonial plugin ? I am looking for a plugin that allows visitors to add youtube video as testimonial or post written testimonials with photos. I have a wordpress plugin , but that allows only 2 fields on the form page ( Testimonial submission form ). I need the form to have 4-5 fields. It can be either free or paid. Does anyone know of any such plugin ? Thanks.
