Wordpress Theme Help needed

I have designed my own theme but there are few issues, I couldn't attach my credits with it. I try to upload theme at themes.wordpress.net but I couldn't and no body is replying back when I contacted them twice.

How I am going to attach description and my credential with the theme?

 

what do you mean? like in the footer at the end of every page of your blog?

 

just modify the footer.php file manually with your credits

 

I mean when you see the theme from administration; under presentation menu. there is some description and name of the creater is displayed for a theme but I can't find how to add my name there.
