The one in the screenshot looks like an ajax newsletter subscription plugin that comes inbuilt with the Aweber newsletter management account. you can find plenty of free substitutes or this kind of plugins, below are few alternatives to persuade users into subscription. i think what you see there is an invite into a mailing list, not register as a user on the blog. "if" it is a mailing list opt in the process would be like this: you subscribe to email list you get email telling you where to download you then get more emails daily/monthly/yearly from this person until you "unsubscribe" from the list. "if" it is to register you could do something like this: say you can get the free report when you register for the site provide the register link to your site (wp-login.php) then have a memebers only page (many plugins can do this) and have the product on this page. the user can then only get the product when they are logged in and it will be invisble to non-registered/logged off users.
You mean subscribe to your mailing list? You should be able to create the form with your auto responder service if you have one. The form will also have something like "after successful sign up redirect to...(url)", and have the ebook at the url or send it in your 1st autoresponder message.
