Buying Wordpress Designer - Javascript dialog with mailchimp form integratio + menu

    Hi guys.

    I need someone who can code and implement into my theme the following:

    Look at the Button that says subscribe, i want EXACTLY the same (look and feel) with a mailchimp form to suscribe.

    Plus i want the user menu to be the same and be coneccted to wp menus.

    Pls ONLY seasoned wordpress designer with great css html and javascript skills.
