Buying Wordpress - Create a Form to Fill out - Need it today

    hello,

    I have a wordpress website and I need someone to create me a form to where people can fill out the information the data will be sent to my email. This new page will be linked within the site.

    The form is quite long and you can see it here: CLICK HERE

    You don't have to have the format this way as long as they are ordered like that. You also do not need the signature section on the bottom. You can install any plugin that you need to do this.

    I need this work today and need it fast. I have all the information on the site that I can give you.

    Email me your COST and WHEN TO start and COMPLETE.
