wordpress.4 theme error

    I have upgraded the wordpress version to wordpress.4.

    But now admin area uploader not working.
    check the screenshot

    Its working when i activate other theme.

    Seems the problem with my modification in that theme.

    Anyone know to fix this ?

    Iam using Transcript wordpress theme
