Will pay $7 to anyone who can incorporate list with sublists into our Form

    The form is ready and working, we just need to add in a list where one option is required to be chosen upon registration. The final list will be very long so I want to incorporate sublists to make it easier to make a choice.

    The form is in html form with corresponding PHP page.


    Please help me with the basics to the list and make it work with the form's submit button.

    I can expand on the list later by myself.

    Finally got some help already.

    Still wish I could find someone close here in Vietnam to brainstorm with and work on code with.

    BTW used freelancer.com before to get some stuff done. I learned a bit through doing that... but it helped. Just keep in mind you get what you pay for, period.
