Will pay $20 for someone to guide me through installing a database

I want to install a database on my server. The database is a .csv file and seperated into state,city,address, etc. I also need to have a simple site designed around that. I will pay $20 for someone to successfully walk me through that.

 

Three steps can finish this work by one time.
1. login the phpMyAdmin on your webserver.
2. create a table that contains all fields needed.
3. click the "import" link and select your .csv file, then click "Go", all is done.

Ok so say I only have an index page up. That lists by state. Similar to http://tinyurl.com/3564ls. How do i create all the extra pages and seperate them by city, etc.

I am not advertising at all. That is an example of the type of site that I want. Just simply showing what I want.

 

if this is just the database installation then I can create. there is no time for me to design the site.
however you can try yourself creating database from phpmyadmin and importing the csv files (if SQL structured)
