    Is there not a link in the email you received from Google?
    If not, simply login to your Google Account, click on my account and click on Adsense - You may need to re-apply from the beginning (ie fill out the form again)

    maybe you missed some other guidelines such as the privacy policy that google requests publishers to have on their web site now
