Who can convert proxy template php --> glype 1.1

Looking for a quick turnaround. The guy advertising the conversion as a service seems to have better fish to fry, but he kindly posted his method.

I can do large parts myself, but a seemingly small problems keeps me from completion. Anybody interested?
