http://www.fourquotes.co.uk/accountants is the best as it shows clearly that privacy is thought of. I also shvows the flow involved in submitting you information. The other forms does not in the same way let you know what happens after submitting.
And make a SHORT video telling the visitor "what's in it for him" - this will establish you as a human being and build trust. Trust the what you want.
The thing is with the accounting form, people tend to get bored with a 3 page form. After they see all the questions they tend to bounce. Remember I am focusing on getting leads in, when I have them I can explain the process. Where can I put the video?

You could capture their e-mail address in step 1 and if they abandon, you can remarket the service through the mailing list.
