What the? I didn't get what you said.

 

I think the second is better, for the first one is directory deeper than the second, and you need set the subdirectory default file type like: index.htm! If you use apache you need set the indexs option!

 

.html, .php, whatever.... is academic. It does not matter. But /category is often not required. It really depends on what you are trying to do. An example of what you are doing would help us answer your query properly.
