Not sure it is clear what you are looking for...
I have KY with max CPC of $0.05 and some with $5
the profitable ones come from all over the range....so
what you are looking for exactly?

No, I rarely get charged max cpc, but I'm dealing with non-competitive keywords.

Usually we are charged the medium between the minimum CPC and the maximum CPC.

Varies. But as said above, usually you get charged somewhere in between.

Just make sure your CTR is going up (how? a science of itself) and your CPC will be down

The adwords discounter works automatically in any case...One still will want to set a Max CPC per the personal ROI

Actual cost-per-click (CPC) is the amount you'll actually pay for a click on your ad. The AdWords Discounter automatically gives you the lowest possible price in order for you to maintain your ad's position. Your actual CPC will be equal to or less than the CPC bid you specify for your ad group, keyword or placement.

Your maximum cost-per-click (CPC) is the highest amount that you are willing to pay for a click on your ad. When ads appear on the search network, the maximum CPC is one of the factors affecting ad position. Increasing your maximum CPC can improve the position of your ad. When ads appear on the content network, the maximum CPC is a key factor in determining whether your ad is placed on your targeted placements. Again, increasing your maximum CPC can improve the chances that your ad will appear.
