You can always opt out by not accepting image ads. But it should benefit most publishers if you can give it a lot of impressions. Curiously, they don't seem to show up on my more popular sites, just the smaller ones. Bummer.

As far as I know from studying these ads behavior: 1. They show on homepages only 2. They don't show for all niches. They do not show for webmaster related, finance or health. They show for forums and business (trading). 3. They are not rewarding. I think they give the same as any other ad.
The CPM ads work in the same sense as Adsense's CPC process -- that is, if the CPM pays more than the CPC slated for that spot, the CPM wins and takes over. This way, Google maximizes their publishers' profits because, obviously, it maximizes theirs. Google likesy the money. If you're getting nothing but this ad, then your stats should be close to the CPM rate. What is it? (I don't think it matters if you tell it, since it's not specific to you, but to a lot of other publishers...)
Not true. On one of my sites, it shows on the front page, archive page, and individual articles. As for their reward, they should, theoretically, be higher paying than what would normally go in their spot. At least, that's the theory.
