What to do if client changes requirements midstream

    I was wondering if anyone had any advice on what to do if a client changes or adds to requirements during a project. Below are four example scenarios, your feedback is very much appreciated (please feel free to answer them individually or just in general):

    Client:
    I just want to make sure that XX feature will be on the website. It is very important for us to have this feature (even though this is the first time I'm mentioning it because I assumed it would be included).

    Or -

    Client:
    We decided to support XX charity rather than the current charity (which I mentioned we would support previously). Can you please change the banner design on the homepage?

    Or -

    Client:
    (I know the homepage is designed and I approved it, but) We decided that we need to add this other graphic somewhere on the homepage

    Or -

    Client:
    I have another design change for you before I approve it (this being the millionth and one)

    How would you deal with these? Would you charge or roll with the punches. If you did charge, how would you respond to their request while still keeping them happy?

    I'm in web design and development but I'm sure many professionals get similar requests and your response will be relevant, useful, and appreciated.
