1. I would do both. Have written instructions and a voice reading them at the same time. 2. Depending on the face. 3. A short, tasteful introduction (15-20 seconds). 5. No longer than 4-5 minutes. My 2 cents.

Do you prefer to see a persons face rather than just their voice? - Person's face and his voice. It's one way of knowing and understanding what the person is talking about. If they prefer instructions with voice, that's acceptable. Much better than having none. Introductions, yes or no? if yes how long? - Yes, 15-20 seconds. I hate low quality videos that people seem to put very little work into. I also don't like it when people copy videos.
For business purposes video, s much as possible 2-3 mins. video clip will do And also, any video will do as long as it isn't low quality and the length will not take 30 minutes to 1 hour. I would love it if it is divided into different parts instead of compiling it in one video.
Music is good but when the person has a low voice and the music is overwhelming, its distracting. Also when it seems as if the person is too close to the speaker and you get more of a whisper than their actual voice but I get through it if its interesting enough.
