I have a medical transcription related website and we are promoting website before 6 month. Ranking is good with all the keywords because we are continuous doing seo work. But sadly our website ranking go down day by day we don't have idea what the exact reason

If its continuous fall, it seems your competition is over taking you. Medical transcription niche is very competitive, so leading such a niche is quite a tricky task. well, once you say your ranking is good and then you say your ranking going down.

if your ranking was good and then it started to drop, then there could be various reasons for it. when you start doing SEO for a site, due to having new content and fresh SEO, you get a ranking boost. however, that initial boost doesn't always remain steady and you will notice your ranking dropping. sometimes it might even take a serious nose dive. this is totally normal and nothing to be concerned about. your ranking will slowly increase with continuous SEO wirk so keep doing what you are doing. you need a lot of work to maintain a stable ranking.

There is some various reason that make your SEO go down day by day but in my experience,there was a rumour that google have a discriminated policy toward some non US/European domain,especially with the third world countries

 

I think the breakdown of the site's rankings is the lost backlinks and the regular updates in the website's content because Google is more on focusing in the quality of content and backlinks that points into your site.
