show the person that you know something about them and demonstrate that you truly are looking out for their best interests as well.

You don't have to follow a set format. Show competence, be upfront and honest. Sounds easy, but I've never received a link exchange email that fits this criteria.

Tell them where exactly their link will be placed and don't bother with link exchange unless your site and your target site are complementary.

Why pester people for links and waste your time sending a ton of emails that will never even be read? There are tons of link exchange sites where people are sitting there WAITING to give you a link!

    Yes this is there are
"are tons of link exchange sites where people are sitting there WAITING to give you a link!" but the thing which is to be considered is that weather those sites are relevant to your sites theme. And also what sought of content does it has.
You must avoid those link exchange sites , Because they can any time be declared as spam and also placing your site link on such a page which has many links on it is not a good approach.

Well, I think that clear text and a little bit of information about SEO and how a Reciprocal Link would be good for each others.

I started to write these kind of emails just today. See how I wrote mine:

    Do you think it's good?
