What plugin to use for WP to have an opt-in form?

    Hi, guys,

    I am new to blogging. Could you please let me know which plugin to use to have optin forms on wordpress blog?

    Preferrably, the optin box shall be customized to be on the pages of either
    1) index page (squeeze)
    2) on side bar
    3) at the end of every post
    4) exit pop up.
