What kind of Adsense Ad format is this?

    I haven't seen this before.. maybe some else here knows more about this special format...

    Yes. Surely a premium publisher. They have various custom formats available with them with are not available with regular publishers.

    Usually, weird looking ads out of the ordinary are premium publisher ads.
    Hope to have one on my sites one day.

    a premium publisher also can remove "ads by google" in their ads.
