    It is a virus.
    2 the ways:
    1. Infected server.
    Checked by login cPanel to create a folder and create a blank index.php file in that folder (via File Manager, don't upload from your PC). after 24 hours, viewing the file. If file was inserted iframe --> the server has been infected. Contact server owner.

    2. If not (1): Your PC was infected. You must:

    Overcoming: Backup before do this:
    - Using the latest Antivirus to scans your local PC and make sure the virus has destroyed or use the best ghost or install the windows.
    - Download the entire code on your web.
    - Install the program: Search And Replace abacre.com/afr.zip
    Set as in the image below and click on Excute
    - Re-Upload items just modified
    Hope successful

    Removed all iframe on your page . Then check again ! Fix Chmod for up security

    Thanks for the tips.
    I am using VPS and since the past week I have similar issue.
    I upload blank index.php/index.html/index.htm all been inserted iframe.
    I did contact hosting provider they advice for change admin password for the script and cpanel pass ect..
    done that same result..
    change chmod to 644 to index.php also same result..
    I did use malware & antivirus provided with vps also clean..
    what next?

    Maybe infected rootkit on server. Remove rootkit
    if you don't know how to remove it. Step by step following:

    Now, backup your code and database to your local PC.
    Rebuild your VPS and sure your VPS is "Clean".
    Re-upload and import database.

    But the visitors visit your site will be infected virus and you site may be blocked by google and/or antivirus programs. You visitors will be not come back.
