What is the rule to recreate a Site Map for a site?

    Let me clarify about the problem. I have a site using yoast seo plugin but the problem is when, I am trying to optimize pages title it's automatically showing random title without separator.So far I know the page separator(|) is set.some how I managed to create pages title just putting example: services- or aboutus|.I know it is not the SEO solution's but I had to do. I have created site map and can see the title is not coming that I actually want. and coming double indexed pages for the same URLs.Now my question is :
    1) should I delete entire site from Web master tool?
    2) should I create site map again?
    3) how many times site map can I create?
    4) what is the best method to delete duplicate pages containing same urls?

    I just use the xml sitemap plugin and let it do all the work, never have any problems!

    well bro you dont have to do any thing,,, for now solution to this problem i will tell you..
    1st delete older site map..
    2nd create and index sitemap
    3rd there is a option in google webmaster... >crawl>fetch as google.. just paste the url where you posted or updated new title and within 24 hours your new tittle will be updated
