When you optimize your site for a keyword you have to optimize your content for that keyword in a 100% so you get the correct density for that keyword and you keyword placed in your link, tittle, h1 tag, alt text... This will represent the 10% of the factors for ranking in Google. The otrer 90% is based in off page optimization by creating backlinks to your site. It is possible to rank an image for a keyword just by sending backlinks to it. In this case you will avoid to optimize the 100% of your optimization for your keyword because you will be penalized. You have to simulate a natural linking so send a 40% of your backlinks with anchors as "Click here", "Thins link" and so on.
That's a good way to get started, but at some point you'll have to start worrying about off page optimization, and start a link building campaign if you want anyone to be able to see your content.
Offpage is simply the things that are not onpage i.e things that are not done on your site but on different sites mostly concentrating on link popularity.
