i haven't heard this term keyword difficulty yet in Seo world ... i think all issue that we cant solve with keyword are keyword difficulties ..

I never heard about keyword difficulty,but i know about keyword density.I think problems related to optimize a keyword are called as keyword difficulty.
keyword difficulty in my opinion means how hard it is to rank for a particular keyword. Most of the time you will see that your True competition is the first 10-20 results.
bob25 listed all of the factors that go into determining keyword difficulty. Some people use shortcuts like number of competing pages or average Page Rank of the first 10 SERPS to determine difficulty and this works fairly well. It's better to use a better tool that analyzes all of the factors to be more accurate. Micro Niche Finder is an excellent paid program and Traffic Travis is a good free program.
