what is causing this 'extra' tab in my theme options [wordpress]

i made my own theme, based off of cutline. the theme itself is completely different, i changed the basic info in the style sheet, and put everything in a new folder.

any idea what is causing this extra tab? i'm guessing it's one of the .php files in the theme folder but i don't know which one.

is there a plugin called cutline or something that you can deactivate maybe?
