What is best to use for questionaire?

    Hopefully this is OK for my post.
    I need to put a questionaire on a site with about 10 or 12 questions on it and the room to answer each one of them. Some answers will be short and some may be 2 or 3 lines long. What would be the best thing to use to do this. I imagine there is a ready made form that someone knows about.

    if you will do not use automated format it will be ok. but you can try.

    I don't understand what you're saying. Where does automated format fit into a questionaire? Are you talking about a specific form? If so what is it?
