What do you guys use to make php forms on html websites?

    I need to make myself a custom form page where customers fill out a form and click submit. Once submitted, it takes them to a webpage that is set in the code.

    Is theres such a program out there or do i have to do it all custom?

    heres an example of my current form, i use jotform for people to send the emails to me, byt thye only have a limit of 100 a month and i would prefer to run the script using my own servers.
