What actions must we avoid to keep from getting deindexed in Google?

Nothing, you only get deindexed for being a dumb ass.

Seriously dude, I'm asking for real. I'm about to start link building and I don't want to be thrown into the sandbox. I've heard that getting too many links to fast can get you sandboxed, is this true?

If you are site is new so dont build more than 150-200 link per month and dont try to use any black hat tech like blog comments etc.

no, getting too many links too fast does not get you deindexed so long as the links are seen as natural

One of my sites got penalized due to (which I think) suddenly adding h1 tags.

Previously I didn't have any header tags in my design and it came to me that if I use h1 tags for the titles, it could help me up a few rows but now this happened.. I've immediately removed those h1 tags and waiting for my rankings to come back.

So my advice is don't over optimize your site. Don't be greedy and don't try to fix it if it's already working.

However, I didn't see any side effect of getting backlinks so far, whether be it relevant or irrelevant links.

I think they deindex if your site contains too much duplicate content, spam, etc.
