Website I just built

I Have an issue with a website that I just built when looking at it in Ie7 the form box looks jacked up. and i'm having a problem with submitting this form. please help my website is www.diversepainting.com

Ok i figured out how to get my form to send me an email, but still need help with the layout

 

You are not opening a <label> anywhere at all. Get rid of all the &nbsp; and </label>.

 

Ok Thanks krsix, I've fixed that know I have another problem, when i cache my site in google under st. louis painter, my site looks jacked up, I wonder is that because of css rules and won't that affect my page rank. I was on page 11 know I'm on page 13
