Buying Website Form

    Hello,

    I am looking for a someone to create me a form similar to this page.

    I need to have drop down lists that depending on which option the user chooses changes the rest of the fields below that question. I can't seem to figure out how to do this myself.

    Please PM how much you would charge to do this and how long it would take. Any questions are also welcome. I will provide the names of the fields and drop down boxes to the person I choose to make the form.

    This is done using javascript, it is in fact very easy but a lot of coding! I can help you with this if you like!
