I design .PSD templates, then convert these to valid xHTML and CSS, I prefer to code predominantly CSS layouts, using as little xHTML as possible. I CAN impliment PHP coding, although I myself do not know how to code it, so if you can provide the PHP coding, I can include it. I have created several designs in the past, few are fully coded though (I do have the knowledge and skill to code them though, just had no real need to). My prices start at $40USD for a design and a coded xHTML/CSS site, with 4 pages. any extra pages are $5 each, as long as they use the same design. At this price you get a fully functional site, with everything done so that once completed, you simply need to upload them and everything will run fine, every design is done as to not rely on any specific browser (Tested in every main browser, FireFox, IE6&7, Opera, Safari) and is non-specific to any resolution (Although every design is done with 1024x768 as the predominant resolution). You receive the .PSD name-layered and grouped. all html files and css file(s), the images folder with the images.
