Did you try typing in something like http://www.~yourURL~.com/blabla/ Code (markup): ? It might've been that Google's changing your SERP therefore your root URL disappears temporarily. (normally 1-2 days).
I don't think ban is the correct word, I think he correct word should be penalized...! If you have used some black hat seo method or something, then there are chances that your website might have been penalized by google...!

Yes. It mifht be temporary issue. If after few days your site won't back it's positions try to request for re-indexing from Webmaster tool.
I don't think. Use the option site:yoursitename and check it whether it appears or not. Usually this condition remains for only 1-2 days only.
