Buying Web Push script

I need a 2-part script
1. a javascript which i will put on my site and it will ask people to signup for notifications (on page load) and when they do - will send gathered data to an ajax call (which will save in DB, i will make that part)
2. a PHP that will send a web push notification (of whatever content i provide) when i feed it with that gathered data from previous step
