There are quite a few ways you can go about doing this; a simple Google search for "excel to web" is the best way to view all the available methods. Beware, you'll still need someone basic HTML\PHP knowledge to do this successfully. Best of luck!
I can tell you to tool named Free Form Builder where you have to fill the fields. It will automatically gets converted into a form. You can also 'Enable E-mail Support' option if you want to receive mail from your user when they submit the form.
