Want to add a search Bar in middle of the page in Wordpress

    hello

    I have a wordpress blog in which, i want to add a search bar at the middle of the page. My blog address is http://quicksurvivalkits.com/.

    for example of search bar, i want to add a middle page search box like this

    How can i do this? please tell me the code and where to place it.
