W-9 Forms

    Hello All,

    I formed a single member LLC for an internet marketing business last month. I signed up for CJ (Commission Junction) and they asked for a W-9 form.

    Here is some information about the LLC:
    1)The LLC is a single member LLC and has no employees.
    2)I have an EIN for the LLC.
    3)I have not filed form 8832.
    4)Total revenue for 2008 should be less than $500.

    Does anyone have any advice on completing it correctly?

    A W-9 form is simple. It's pretty much just your business name and EIN, plus your name and signature.
