Video is not showing up

I need help, video is not showing up... I add it, we can see it in home page, but when we click on it, it doesn't play. (Here it is: www.allxxx.tv [It's adult content])

So as I scanned I guess problem is here:

This is add_video.php which is done from admin panel.
