Very simple job available in Word

    I have an eight page questionaire which requires reformatting and reducing to five pages. (see attached)

    It needs to be more presentable to include text boxes arround the questions and check boxes alongside choices where appropriate.

    pm me if interested with a quote (nice and cheap please)

    It would be beneficial if you could provide a sample. thnx
